ImageAssist is looking for a Healthcare EMR Engineer to join our growing team and help lead the development and expansion of our clinical platform's EMR connectivity layer.


ImageAssist is a modern clinical photography platform used by healthcare providers to capture standardized patient photos, improve documentation, and seamlessly integrate images into the medical record — all while saving time, reducing risk, and growing revenue for providers.

ImageAssist Overview

ImageAssist is a clinical photography platform designed to standardize and simplify how patient images are captured and used in healthcare. Today, clinical photography is fragmented — often captured on personal devices, inconsistently framed, and disconnected from the medical record.


ImageAssist solves this by:

  • Enabling clinicians to capture high-quality, standardized photos using iPhone or iPad
  • Providing smart guidance tools to ensure consistency across time
  • Automatically organizing and storing images in a secure, HIPAA-compliant system
  • Integrating directly with EHR systems such as Epic and Cerner

By improving how images are captured and managed, ImageAssist helps providers enhance clinical documentation, improve patient communication and outcomes, streamline workflows across teams, and build high-quality before-and-after photo libraries.

About the Role

This role is ideal for an entrepreneurial engineer who understands the complexity of healthcare data infrastructure and the importance of reliable, secure EMR connectivity. You'll own the EMR connectivity layer end-to-end — auth flows, FHIR clients, data normalization, and the APIs our mobile and web apps depend on.


This is a contract (1099) role, where you will leverage your technical expertise, supported by the ImageAssist team, to build and expand integrations across our target EMR ecosystem.

Responsibilities

  • Become an expert on ImageAssist's platform architecture through onboarding and independent study
  • Design and ship FHIR R4 integrations against Epic (App Orchard / Open.Epic / Showroom), including SMART on FHIR Standalone Launch, token refresh, and scope management
  • Extend EMR coverage to Athenahealth and Oracle Health (Cerner Millennium), plus our existing ModMed, Symplast, Mindbody, and FourD connectors
  • Build clean, typed Express endpoints for appointments, patients, practitioners, sessions, and clinical document upload (DocumentReference)
  • Harden multi-tenant token storage, rate limiting, retries, and FHIR OperationOutcome error handling
  • Partner with mobile and web teams to model a unified API surface across heterogeneous EMR backends
  • Collaborate with the ImageAssist team on feedback, technical positioning, and integration insights
  • Drive adoption of a platform that meaningfully improves clinical workflows and patient care

Requirements

  • 3+ years in Node.js + TypeScript building production REST APIs
  • Hands-on experience with Epic and SMART on FHIR / FHIR R4 (Bulk FHIR a plus)
  • Strong grasp of OAuth 2.0 / OIDC, JWT, refresh-token lifecycles, and PKCE
  • Comfortable with Express, async HTTP clients (axios), and containerized deploys (Docker)
  • Familiarity with clinical workflows, documentation, and EHR systems
  • Experience navigating healthcare API sandboxes and production go-live processes
  • Strong communication and collaboration skills across technical and non-technical teams
  • Self-motivated, entrepreneurial, and able to operate independently

Nice to Have

  • Direct experience with Athenahealth or Oracle Cerner APIs, or USCDI / HL7 v2 backgrounds
  • Epic App Orchard / Showroom registration and sandbox onboarding experience
  • Open.Epic sandbox testing, Epic Vendor Services, or production go-live with a health system
  • Firebase Auth / Firestore, Stripe, and observability tooling (Sentry)
  • HIPAA-aware engineering: PHI handling, audit logging, BAA-bound infrastructure

This is a 1099 contractor role with compensation tied to scope and performance.
